If your children in grades K through 7 DO NOT attend Catholic School,

please enroll them in our parish’s Faith Formation Ministry.

Space is limited. Children are placed on a First Come, First Served basis according to the order the application is received.

Faith Formation Registration Form

Please note: K/1st Grade Catechesis of the Good Shepherd classes are limited in class size. Thank you for your understanding.

    Please note: Registrations are accepted in the order they are received, and classes are filled on a first come, first serve basis. Please select just one of the options below for your child. In-person Formation is preferred. If the options below are not feasible, please contact us directly for more information regarding the possibility of an online formation option, which is available to students in grades 4th through 7th only.

  • Additional Student Information

    If your child is receiving his/her sacrament of First Communion this year, we need a copy of his/her Baptism Certificate. You may visit our parish office, or email us a copy of it. If you are unable to locate it, please contact the parish where your child was baptized to request a copy. If your child was baptized at St. Jude, we should already have it on file.
