If your children in grades K through 7 DO NOT attend Catholic School,
please enroll them in our parish’s Faith Formation Ministry.

Space is limited. Children are placed on a First Come, First Served basisaccording to the order the application is received.

We offer either Catechesis of the Good Shepherd (Level 1 Ages 3-6 or Level 2 Ages 6-9).
Catechesis of the Good Shepherd (CGS) is a Montessori-based way of learning within an environment called an Atrium.

We also offer a conventional classroom for the lower grade levels (the traditional classroom model for grades K through 5).

For 6th and 7th graders, we offer Small Group Discipleship classes which include both adult and peer mentors.

Small Group Discipleship is also offered for 8th and 9th graders who are in Confirmation Formation.

PERMISSION AND MEDICAL TREATMENT WAIVER – Please read carefully before answering.*
I do hereby give my permission and consent, as defined below.
As this child’s parent/guardian, I do hereby give my permission for him/her to attend St. Jude religious education classes and functions and to be treated for a medical emergency in my absence while participating in the Religious Education Program. The Director of Religious Education or Adult Supervisor may act as an agent in my absence. In case of an accident, I do not hold the Diocese of Palm Beach, the parish of St. Jude Catholic Church, any of its staff or volunteers responsible.
Additional Student Information
If your child is receiving the sacraments of either First Communion or Confirmation, we need a copy of the Baptism Certificate, as well as the First Communion Certificate. You may visit our parish or email us a copy of it. If you are unable to locate it, please contact the parish where your child was baptized to request a copy. If your child was baptized at St. Jude, we should already have it on file. Tuition Donation for children not enrolled in a sacramental prep year is $50 per child. If enrolled in either First Communion or Confirmation Sacramental Formation, Tuition Donation is $150 per child. We accept cash or check made payable to St. Jude Catholic Church.
